Christian County Health Department will be at the Ozark Schools the week of August 10th-14th, 2026. 8am-11am


August 10th: Ozark Middle School 8am-11am

August 11th: Ozark East Elementary 8am-11am

August 12th: Ozark West Elementary 8am-11am

August 13th: Ozark Jr. High School 8am-11am

August 14th: Ozark High School 8am-11am


The immunization form must be filled out prior to receiving the immunization(s). You may print and fill out the form or they will be available at the time of service.

Kindergarten Vaccines: 4yrs-6yrs | DTaP, Polio, MMR, Varicella | Recommended: Hep A

8th Grade Vaccines: 11yrs-14yrs | TDaP, Meningococcal ACWY | Recommended: MenB, HPV

Senior Vaccines: 16yrs+ | Meningococcal ACWY | Recommended: MenB, HPV
